Step 1:  Check Your Inbox & Set Up Your Portal

You will receive a welcome email confirming your subscription. This email will contain a unique link to create your password and activate your secure patient portal.

Step 2:  Complete Your Health Profile

Log in to your new portal to securely fill out your medical history and new patient forms. Completing this beforehand makes your first visit more focused on you and your goals.

Step 3: Schedule Your First Visit

Once your profile is complete, you can easily book your comprehensive "new patient" appointment online through the portal or by giving our office a call.

Step 4:  Meet Your Doctor & Create Your Plan

During your initial visit, you and your physician will get to know each other, review your history, and collaboratively build a personalized care plan for your long-term health.
